The mental health care services that we provide are in high demand and this policy is to help ensure that we are fair to all members and families needing services by outlining our expectations in regards to your scheduled appointments
How to Cancel and Reschedule Appointments
We understand that things come up and schedules change and sometimes an appointment needs to be canceled or rescheduled. If you need to cancel or reschedule an appointment please call our office as soon as possible so that we can offer your spot to another member that needs care. You can reach our scheduling department by calling (800) 516-0975 ext. 2.
No Show/Late Cancellation/Late Reschedule
A no show appointment is defined as a scheduled appointment that the member does not show up to without any notice given.
A “late” cancellation or reschedule is an appointment that is canceled or rescheduled with less than two business days of notice. For appointments scheduled on Monday, notice must be given by no later than the appointment time on Thursday of the prior week.
In an effort to minimize wait times for members trying to get appointments, Bend Health reserves the right to:
- For collaborative care (CoCM) members, accrue time spent with regard to clinical related activities to ensure the member is staying on track with the care model prescribed, or
- For all other members, issue a fee for the late cancellation/reschedule or no show appointments. This fee may not be reimbursable by your insurance company, and will be billed to you directly. The fees are as follows:
- BCM or Coaching session - $50
- Therapy session - $75
- Psychiatry session - $175
After 3 no-shows, late cancel, or late reschedule appointments, Bend Health maintains the right to terminate its relationship with you.
I understand the no-show policy of Bend Health and agree to allow Bend Health to charge my credit card on file the fee for any no-show, late cancel, or late reschedule appointments. I understand that I must cancel or reschedule any appointment at least 48 hours in advance in order to avoid a potential no-show charge to the credit card provided.
